ANSER* Questionnaire
We use the ANSER* Questionnaire for parents, teachers and students over 9 years of age.  These are very useful in organizing and detailing background information as part of our assessment.  You may stop by and pick up the forms or if you'd like, we will mail them to you.
 
   
 
STRANDS
The Survey of Teenage Readiness and Neurodevelopmental Status may be self administered or provided as part of a structured interview with your adolescent student.  It is based on the neurodevelopmental approach to learning differences and the evolving abilities of adolescents to think about thinking (metacognition).
 
 
   
 
Brown Attention-Deficit Disorders Scale
This  questionnaire offers support in understanding and differentiating attention control problems including an excellent assessment of executive functioning.  We offer this along with the ANSER* Questionnaire above when organizational and attention problems are predominant issues.
